Chimeric Antigen Receptor T-cell (CAR T) technology transformed cancer treatment, presenting remarkable opportunities while also grappling with challenges such as high costs, adverse reactions and difficulties in addressing solid tumors. Dedicated researchers are striving to improve CAR T therapy, with the aim of broadening its availability and bolstering safety measures to benefit a wider range of cancer patients.

At the forefront of driving this change is Miltenyi Biomedicine GmbH, a pharmaceutical company committed to advancing and commercializing cell and gene therapies for severe diseases like oncology with unmet medical needs. With a primary emphasis on oncology indications and expansion into autoimmune diseases, its therapies are set to revolutionize medical treatments.

Miltenyi has a three-decade track record of in-house drug development capabilities, including the production of essential components like reagents and devices required for CAR T cell production. Its state-of-the-art production facilities in Germany and the US adhere to rigorous regulatory standards and maintain Good Manufacturing Practice (GMP) approvals. Equipped with world-class machinery and distinct clean room areas, these facilities ensure precision, reliability, and safety in CAR T cell production, without the need for an external partner.

Among Miltenyi's numerous accomplishments is the development of zamtocabtagene autoleucel, a cutting-edge CAR T cell therapy. Presently undergoing late-stage clinical trials for relapsed or refractory diffuse large B-Cell lymphoma (DLBCL), this unique dual-targeting CAR T cell therapy addresses CD19 and CD20 antigens on B cells. zamtocabtagene's use of fresh cells ensures a rapid production timeline, further enhancing its impact in changing the healthcare landscape.

“While trials are ongoing, our preliminary data from the U.S. trial interim analysis of zamtocabtagene indicate positive safety outcomes,” says Linda Hanssens, head of clinical research and development at Miltenyi biomedicine.

The firm’s approach focused on safety and positive outcome of its trials, addresses treatment gaps in specific medical indications, particularly advanced cancer with limited therapeutic options. Miltenyi is committed to accelerating the development and delivery of its therapies, aiming for a rapid two-week turnaround from patient-derived material to the administration of modified therapeutic products. For patients who have exhausted all available treatments, the prompt delivery of a solution is crucial.

Miltenyi's preclinical division plays a significant role, evaluating potential therapeutic targets while emphasizing technological advancements and addressing healthcare requirements. With approximately 16 to 17 planned and active trials, including those in Phase 1 trials for haematological malignancies and solid tumours, the firm’s portfolio reflects its dedication to innovation and advancing healthcare. Currently, the team at Miltenyi is leveraging its internal infrastructure actively formulating a global commercialization model.

Besides cancer treatment, Miltenyi is also venturing into regenerative therapy research, aiming to restore damaged tissues and organs. Projects include the use of stem cells to tackle conditions like neurodegenerative diseases and diabetes in the elderly. Additionally, the company explores gene editing and delivery technologies to correct genetic defects and enhance cellular functions, showcasing its commitment to advancing medical science.

Looking ahead, Miltenyi is working towards decentralized point-of-care manufacturing, a potential paradigm shift that could bring therapies closer to patients. While current regulatory requirements dictate centralized production timelines, the company envisions a future where production becomes decentralized, ensuring more accessibility of life-saving therapies.

Miltenyi's founder Stefan Miltenyi’s over three decades of industry experience and active involvement to improving the pharmaceutical sector propels the firm’s ongoing research and development efforts. This unique combination of visionary leadership and scientific knowledge positions Miltenyi as an organization uniquely equipped to drive advancements in healthcare.
