Saliva as the New Front Line in Reimagining Diabetes Screening at Global Scale by Pop Test LLC

Randi Altschul, Chief Executive Officer , Pop Test LLC

Noninvasive Diagnostics Pathfinder

Editor’s Note: Healthcare leaders must rethink screening models as scalability and accessibility become critical in addressing global chronic disease burdens. This perspective underlines how noninvasive diagnostics can unlock earlier detection pathways while reducing systemic barriers to population-scale screening.

Pop Test LLC’s saliva glucose platform is built to do something conventional blood tests and wearables cannot: deliver accurate, non invasive screening at population scale, with a stepwise pathway from simple visual yes/no results to full quantitative readouts all on the same saliva based technology backbone.

A Stepwise Platform From Visual to Quantitative

The Pop Test Saliva Diabetes Platform launches as a qualitative screening test—positive/negative at a defined saliva glucose cut off aligned with established blood glucose thresholds for diabetes—readable with no electronic device. This enables immediate deployment in low resource settings where health workers can screen large numbers of people using only low cost strips. The roadmap advances to a semi quantitative format (low/normal/high via smartphone app or color scale for rapid risk stratification) and ultimately to a fully quantitative mode using smartphone imaging to convert reflectance values into blood equivalent glucose, bringing numerical precision while retaining saliva sampling’s core advantages.

Why Glucose Screening and Why It Works

Over 50 years of research and 538 cumulative peer-reviewed articles (as of 2024) confirm that salivary glucose reliably tracks hyperglycemia. Pop Test’s published Clarke Error Grid analysis placed 100 percent of results in Zones A and B—the clinically accurate and benign-error zones—validating its safety for screening use. In 2025 validation (N=175), a saliva cut off calibrated to the 126 mg/dL plasma diabetes threshold yielded 97.4% sensitivity, 97.97% specificity, and positive and negative predictive values of approximately 97–98%. The reagent technology is protected by issued US and international patents covering formulas and manufacturing process, securing exclusivity through the early 2040s at a manufacturing cost of just $0.02 per test pad.

A Global Epidemic Demands a Scalable Solution

The IDF 2025 Atlas reports 589 million adults living with diabetes worldwide—252 million undiagnosed—projected to reach 853 million by 2050. Existing solutions remain inaccessible to most of the world: CGMs run approximately $10 per day ($3,600+ annually), flash glucose monitors approximately $3.60 per day, and conventional SMBG strips $0.80–3.30 per day. Pop Test targets <$1.00 per day in the US market, with lower international price points, making mass public health deployment genuinely viable.

India and US Dentistry: Two Compelling Entry Points

India—with 101 million diabetics, 136 million pre-diabetics, and 57% of cases undiagnosed—is the archetypal deployment use case. A visually read strip requiring no reader, no electricity, and no cold chain can scale across community health camps, schools, and rural clinics, identifying likely-hyperglycemic individuals and routing them to confirmatory care. In the United States, the same technology supports chairside screening in dental practices, where hygienists can flag patients with likely high glucose during routine visits without disrupting existing workflows.

A Multiplex Future: Uric Acid, Cholesterol and NASH/MASH

Glucose is the spearhead, but the Pop Test platform is engineered as a multi analyte system. The same sampling and readout infrastructure extends to uric acid for gout screening (affecting approximately 8.3 million Americans), cholesterol and lipids for cardiovascular risk assessment without fasting blood draws and NASH/MASH markers—providing a non invasive front door to fatty liver disease detection in a population where up to 25 percent may have NAFLD.

Seeking Strategic Partners

Pop Test’s model is to develop, validate and patent—then out-license or partner with established diagnostic, pharmaceutical or retail healthcare companies for global commercialization. The company is actively seeking strategic partners to advance through FDA 510(k) clearance and into full commercial deployment. With validated accuracy exceeding 97 percent, a strong IP portfolio, $0.02 manufacturing cost and a platform adaptable to virtually any environment, saliva based screening is positioned to become a practical new front line in the global fight against metabolic disease.
